I understand and agree that Forward Motion may use, reproduce, display, distribute, publish, and otherwise utilize the Media, in whole or in part, for lawful purposes including, but not limited to, marketing, advertising, promotional materials, educational content, fundraising, social media, websites, blog posts, print materials, and other online or offline media, now known or hereafter developed, without further notice or compensation to me or my Child.

I understand that the Media may be edited, altered, combined with other materials, or used in different formats, provided such use is not defamatory or misleading. I waive any right to inspect or approve the finished Media or the manner in which it is used. I further waive any claims against Forward Motion related to the use of the Media, including claims for invasion of privacy, right of publicity, or misappropriation, to the fullest extent permitted by Montana law.

I am the parent or legal guardian of the child identified above (please type name of child/children participating). I hereby grant permission to Forward Motion, to photograph, videotape, audio record, or otherwise capture my Child’s and my own likeness, image, voice, and appearance (collectively, the “Media”) in connection with participation in any group, program, or event hosted by Forward Motion (“Event”).

This consent is voluntary and remains in effect unless and until revoked by me in writing. Revocation shall apply only to Media captured after written notice is received by Forward Motion and shall not affect Media already created, published, or in use prior to revocation. I represent that I am at least eighteen (18) years of age and the legal parent or guardian of the Child, and that I have the authority to grant this consent on behalf of my Child.
